Service Policies

• When scheduling an appointment, please understand the Practitioner's time is valuable and that canceling your appointment within 24 hours of the scheduled time may result in an outstanding balance, valued at the same price as the scheduled session.

• All Packages must be paid in full and expire 12 months from the purchase date. Packages can be shared among friends and/or family.

• New Clients will be asked to complete a Client Intake Form and sign an Agreements prior to their first appointment for each type of service. 

• Existing Clients may occasionally be asked to review/update their Client Intake information.

• Prices and availability of products and services are subject to change without notice.

• As of January 1, 2023, Kentucky State Law requires the application of a 6% Sales Tax for all services as well as products.  This tax applies to cash, checks and credit card payments.

*The only exception to this would be the use of a HSA or FSA account. Accounts may be utilized with a script from a current provider.

• Processing fees for credit cards have steadily increased. Please pay by Check or Cash if at all possible.
